Two (2) areas of Botox or Dysport, a $900 value, with Board Certified Plastic Surgeon Dr. Matthew Schulman. The forehead, between the eyebrows (the "11's"), and the corners of the eyes (the "crow's feet") are each defined as one area. This treatment is subject to evaluation by Dr. Schulman. If he feels that this treatment is not indicated based on his medical evaluation, he will allow substitution of a comparable treatment. Appointment required.
