- Must submit certificate on original letterhead and signed by an authorized representative.
- Certificate can only be used one day and for a foursome, not individual rounds on different days.
- Must reserve tee time five days in advance.
- Valid on any weekday (Monday-Friday) excluding holidays.
- Cart fees are included.

Please bid carefully because all net auction proceeds will fund MDA research, summer camp and other services provided by the Muscular Dystrophy Association. MDA accepts your bid as a promise to pay.

will apply and will be billed to your account. All gift certificates (regardless of the number of certificates) will have a one time shipping
charge of $5.95 and all other packages will cost $14.95 unless the weight of the package exceeds 20 pounds, and:
