This certificate entitles the bearer to a pediatric dental examination at the office of Dr. Mary Hayes, Dr. Joanne Oppenheim or Dr. Marilia Montero. The patient for whom the service is performed can be a new or a patient of record. Included are oral care instructions and diet counseling as appropriate, a dental cleaning and a fluoride treatment.
