Experienced geriatric care manager will review the current living situation of the person you present, discuss possible alternatives depending upon health and affordability.  A form will be provided to complete ahead of the meeting to permit the most comprehensive discussion.  If meeting with the elder person is possible, that would be helpful, and would be included.
