Package includes gift bag with coffee mug and dental supplies.
Special Instructions
Does not include any adjunctive services that may be needed by the patient and provided by a general dentist or other dental specialist (such as extractions, jaw surgery and grafting).
If the patient is not ready to begin orthodontic treatment he/she will be monitored until the appropriate time to begin treatment.
If the patient transfers their care to another orthodontist or discontinues treatment, no refund will be given.
The patient will be subject to our normal contractual obligations and office policies.
