This certificate entitles the owner to one Flag Football or Cheerleading Registration at Faith Assembly. You must sign up your 1st through 8th grade child in January 2013 at the church office located at 6950 Bayshore Road, North Fort Myers. Includes registration, player box with jersey or cheerleading uniform and end-of-season gift. Not valid for any other Upward program.
