This certificate entitles the recipient to one complimentary occupational therapy evaluation from Kornerstone Kids, LLC including:

• Up to one hour of evaluation time with your child including any required standardized testing


• Discussion with parents regarding evaluation findings


• Written report documenting the findings of the evaluation. Includes testing scores and goals for treatment.
