The bearer of the winning certificate may not be currently enrolled at the gym for the past 6 months. Certificate may not be combined with any other offers or coupons. Certificate may be used in its entirety or in part at different times but must be used prior to the expiration date. Classes must be used 4 weeks in a row, one day of camp must be used in one day. 45% off in store must be used on one time purchase of up to $200.

Pick up will be at Beethoven Elementary School on Monday, May 6, 2013 and Tuesday, May 7, 2013 from 8:00am - 9:00am and from 2:30pm – 3:00pm. If item will not be picked up from Beethoven Elementary School, located in Los Angeles, CA, an additional $5 fee will be added to cover USPS Priority Mail.
