Please return this certificate with your child’s registration form.
Non–transferable, may not be redeemed for cash.
Certificate valid from May 1, 2013 – May 30, 2014, no exceptions.
Pick up will be at Beethoven Elementary School on Monday, May 6, 2013 and Tuesday, May 7, 2013 from 8:00am - 9:00am and from 2:30pm – 3:00pm. If item will not be picked up from Beethoven Elementary School, located in Los Angeles, CA, an additional $5 fee will be added to cover USPS Priority Mail.
