Patricia Mulvey, LMSW, C-ASWCM is an individual Geriatric Care Manager out of Irvington, NY, who works with families to plan the future or current situation of an elderly parent or relative.  Pat will meet with family members to help identify issues and develop a road map as to what may be necessary for their loved one, either to remain in their homes with help and support or relocate to a facility that will address their needs.
