(Lost or broken retainers can always be replaced for a nominal fee.)

All patients must qualify for treatment and will be assessed at an initial, complimentary consultation. The value of this certificate cannot be divided between multiple patients. This certificate holds no cash value in lieu of treatment. It may not be duplicated or combined with any other offer or discounts. Certificate must be presented at exam and by the expiration date of September 30, 2016. Please note: Treatment does not need to start by this date.
