Included in this package is one set of basic estate planning documents for one individual person. The package includes a Statutory Will, Self-proving Will Affidavit, Power of Attorney, and Healthcare Directive. Also included is a consultation regarding each of the above-stated documents with Attorney Jim Jolly, 21000 W. Ten Mile Road, Southfield, Michigan, 48075. Phone: 313-461-7758.
These services will be provided by Attorney Jim Jolly. Jim has been a licensed Michigan attorney since 2008. He served the Livonia community as long-time Attorney Magistrate of the 16th District Court and presently serves as a member of the Livonia City Council.

This package is not transferable and must be used by no later than November 30, 2017, or become void with no value and initial auction donation price converted to a straight donation to Seedlings Braille Books for Children with no consideration.
