Special Instructions
Certificate must be redeemed by 12/31/2017 and membership is good for one year from the date submitted. Certificate is redeemed by mail and membership materials should arrive in 3-4 weeks. Once membership cards are received, a cardholder must be present on each day of visit to receive access to member benefits.
