A HCP is a document giving a proxy the right to make medical decisions should you be unable to make them yourself for any reason.


 


A Power of Attorney gives your "attorney" (does not have to be an actual attorney...its whomever you choose) the power to make financial decisions for you should you become unable to make these decisions yourself.
