- Coach Class round trip voucher valued at $1250 each

- Vouchers needed to be ticketed prior to their expiration date

- Valid on Alaska Airlines or Virgin America only, not codeshare partners or for flights to Cuba

Certificate is valid for flights served by Alaska Airlines flights numbered 1-999, 2000-2999, Virgin America Flights 1000-1999, SkyWest Airlines flights 3450-3499, and PenAir flights 3275-3299.  Not valid for flights to Cuba or on codeshare partners.  Mileage Plan Miles cannot be earned or credited.  First Class pair upgrades or mileage plan upgrades are allowed.  Standard baggage fees apply; see baggage policy and flight schedule at www.alaskaair.com for more details.  Individual travel taxes may apply.  Once ticketed, fare rules apply regarding changes to itinerary.  Certificate is void if bartered or sold.

Booking Directions

1. Allow two weeks from the time of reservation and your flight.  TO make your reservation, call Alaska Airlines Commercial Desk (daily 5am-12amPST) at 1-888-327-2755.  Inform the reservation agent that you have a Travel Authorization Form along with any inquiries you may have regarding MVP, Gold, or 75K Gold membership benefits.


2. When you have confirmed your flights ask the agent for your confirmation code.  Complete the bottom portion of the Travel Authorization Form letter then mail the entire letter via certified delivery to:
