Description
This certificate is good for one full orthodontics with Wafelbakker Anderson Orthodontics for a child which includes: Full examination, orthodontic records, full braces, visits for adjustments, one set of retainers following completion of treatment. Initial examination must be done within 4 months of receipt of this certificate.
Special Instructions
Excludes: any treatment necessary but performed by another dentist, (ie oral surgery, extractions, periodontal treatment, cleanings, etc.) Please bring certificate to first appointment. Certificate is only applied at the start of treatment, cannot be used towards treatment already in progress.
