Must complete and sign all required paperwork. Must have consultation with the doctor prior to treatment, which may be done on the same day as the treatment. Botox should not be administered to a pregnant or nursing woman. Botox should not be administered to anyone with a signifivant neurological disease including but not limited to Myasthenia Gravis, Lambert-Eaton Syndrome, Amyotrophic Lateral Sclerosis (AML, Lou Gehrig's disease) or Parkingson's disease. Botox should not be administered to anyone with a known allergy to the toxin ingredients or to human albumin. Expires June 2019.
