• cannot be transferred to another person


• must become an established patient within one year of purchase (consultation)


• insurance and medical reimbursement cannot be used with the auction certificate


• no money is transferred through this office; it is strictly a donation on Dr. Long's part to the school or establishment that is holding the auction.


• one year to redeem


• certificate must be present at time of consultation 


• cannot be used in conjunction with Insurance or FSA or Invisalign
