· Bearer must sign documents after reaching age 18
· Bearer will be asked to complete and submit planning form prior to completion of
documents and allow two-weeks notice before documents are needed by bearer for
office to complete documents.
· Bearer will be given option of instructions on signing documents before a notary or
signing at law office with appointment;
