(a) Estate Planning consultation to assess specific needs;

(b) Preparation of Durable Power of Attorney

(c) Preparation of Advanced Care Plan, Durable Power of Attorney for Health Care and Living Will; and 

(d) Preparation of Last Will and Testatment, which will include testmentary trusts for care of minor children, if applicable
