• one complete orthodontic case per patient


• the service is to be provided by Dr. Shawn Long, Orthodontic Specialist 


• one year to redeem; non-transferable


• certificate must be present at time of consultation or new start appointment


• cannot be use in conjunction with insurance, HAS or Invisalign

Includes a Philips Sonicare 2300 electric toothbrush, a size large tie-dye Brace Yourself t-shirt and insulate travel mug

Certificate rules:

1. cannot be transferred to another person/family


2. must become an established patient within one year of the purchase (consultation)


3. insurance and medical reimbursement cannot be used with the auction certificate


4. no money is transferred through this office; it is strictly a donation on Dr. Long's part to the school or establishment that is holding the auction.
