One (1) Neuropsychological, Psychological, or Developmental Evaluation for ADHD, Memory, Learning, Concussion, etc.
It is limited to an assessment for ADHD, mood, learning, autism, developmental issues, memory, and/or sports concussion. Anyone under 18 must be accompanied by a parent for the interview (parents can be virtually present). The intake and testing must be scheduled and completed in the San Diego Office at 3200 4th Ave, Suite 100, San Diego, CA 92103 before September 30, 2024. There will be intake paperwork that must be completed before the first appointment. Testing may be one or two days.
This certificate is not redeemable for cash, money, or other services. It is not transferable to another person without prior authorization by Dr. Ward. Dr. Ward, or colleagues, will choose the assessment tools and assume the right to cancel this for any reason (e.g., ethical/legal reason), with no refund. We may ask for insurance information. You will need to bring an ID for the evaluation. This is limited to a clinical evaluation. It is not to be used in any legal context.

Bidder is responsible for Shipping and Handling fee and any additional insurance, which will be added to the winning bid amount.
