Must complete and sign all required paperwork. Must have a consultation with the doctor prior to treatment, which may be done on the same day as the treatment. Botox should not be administered to a pregnant or nursing woman. Botox should not be administered to anyone with a significant neurological disease including but not limited to Myasthenia Gravis, Lambert-Eaton syndrome, Amyotrophic Lateral Sclerosis (AML, Lou Gehrig’s disease), or Parkinson’s disease. Botox should not be administered to anyone with a known allergy to the toxin ingredients or to human albumin. 

PICK UP / SHIPPING INSTRUCTIONS

Bidder will be responsible for the shipping/mailing fee, which will be added to the winning bid amount. Items will be shipped via USPS and billed at standard industry rates. 

Items cannot be replaced if broken, lost or stolen. Therefore, tracking is advised.

Auction items will not be shipped nor available to be picked up prior to April 8, 2024. 

SPECIAL NOTE: Crespi Carmelite High School will be closed from March 27-April 7, 2024 for Easter Break.
