Restrictions: NEW PATIENTS ONLY. By appointment only. Please call to schedule an appointment. 24-hour cancellation policy; late cancellations are subject to a $50 fee. Must be in overall good health. Must complete and sign all required paperwork. Must have a consultation with the doctor prior to treatment, which may be done on the same day as the treatment. Botox should not be administered to a pregnant or nursing woman. Botox should not be administered to anyone with a significant neurological disease including but not limited to Myasthenia Gravis, Lambert-Eaton syndrome, Amyotrophic Lateral Sclerosis (AML, Lou Gehrig’s disease), or Parkinson’s disease. Botox should not be administered to anyone with a known allergy to the toxin ingredients or to human albumin.
Bidder will be responsible for the shipping/mailing fee, which will be added to the winning bid amount. Items will be shipped via USPS and billed at standard industry rates.
Items cannot be replaced if broken, lost, or stolen. Therefore, tracking is advised.
