This certificate grants one individual a moderate Invisalign case. A moderate case will include 3 refinements (extra aligners) if needed, a set of 4 Vivera retainers (typically lasts 4-6 yrs) and bleaching. Please note this is transferable to another individual. The individual MUST have an initial consult to confirm that they qualify.  At the initial consult the prospective patient will be examined.  If any issues are present, they must be addressed by their current dentist prior to starting treatment. Case must be started prior to 12/31/25.  Please call the office to schedule your initial consult and let the staff know that you are the SE Ball winner.  Winner must present this certificate at first visit.
