I hereby accept the item and understand that the PTA assumes no responsibility for the authenticity, condition, or validity of the description of the item. I understand that the value indicated for the item is a good-faith estimate of the value of the goods/services furnished. I further understand that all items are sold "as is", and that any restrictions noted related to the item are non-negotiable.

By purchasing this item, I release the California State PTA, including all unit, council, and district PTAs and all of their officers, directors, members, volunteers, employees, and agents (collectively "the PTA") and the item donor, from any and all liability, claims, demands, actions, and causes of action arising out of or related to the possession, redemption, participation in, or other use of the item. 

I am fully aware of and assume all risks in connection with the possession, redemption, participation in, or other use of the item.
