This certificate entitles an individual an IntraLASIK,Blade-free all Laser Zyoptix Custom LASIK with Intralase procedure.  This includes:  Consultation to determine if the patient is indeed a candidate; Intralasik procedure for both eyes incorporating Custom LASIK, which uses “wavefront” technology to map specific vision problems.  Mapping allows treatment of nearsightedness, farsightedness, astigmatism, and irregularities unique to each eye; and follow up visits for one full year.
