• Must activate certificate within 45 days of August 30, 2008.  Entitles recipient to two nights of accommodations in a standard hotel


• Accommodations are for two adults 21 years of age or older and up to two chilren under age 18.  No pets.


• Accommodations are subject to availability


• Certificate is valid 12 months from Activation Date or no later than September 30, 2009, whichever comes sooner.  Travel must be completed before the expiration date. 
  


• Reservations require 21-days advance notice.  A $100 refundable credit card deposit  is required  at time of reservation confirmation.  


• Incidental Charges:  This certificate is for hotel accomodations only and does not include transportation, parking fees, airfare, room taxes ($2-$22 per night), meals, and any miscellaneous expenses.  Upon checking in, hotels will require an imprint of a major credit card to cover such expenses.


• Cancellations:  Cancellations or changes to any active reservation must be received at least 72 hours prior to date of arrival.  Failure to cancel or reschedule at least 72 hours prior to arrival or simply not showing up will result in forfeiture of your certificate and deposit. 


• Offer is not valid where restricted or prohibited by law. 

Following e-mail notification of a winning bid, item will be available for pick up at the MDA Waco Office.  If shipment of your item is required, shipping charges will apply and will be billed to your account.
