You will receive one skin resurfacing treatment subject to the below requirements and conditions set by the American Society of Plastic Surgery of which Dr. Katzen is  a member.  Conditions for use of this certificate:

1. The Physician must conduct a medical examination prior to winner having procedure;

2. The Physician is able to refuse performing the procedure at any time if the winner is not found to be a suitable candidate;

3. The winner can refuse to receive the procedure if he/she is not comfortable with the physician performing the procedure anytime during the duration of the treatments;

4. The certificate is non-transferable; and

5. The Physician or staff shall not be exposed to financial recourse if he/she refuses to perform the procedure for any reason.
