Restrictions: Gift certificate must be used by September 2010. May not be used during the months of October and March. May not be used during the Labor Day weekend, Memorial Day weekend, or February 14th.

- Selected items will be available for pick up at the specified MDA Office no later than September 30, 2009,
- Some items will be available at donor locations as noted in the item description, and;
- If shipment of your item is required, shipping charges and insurance will apply and will be billed to your account.
