Membership must be redeemed prior to May 31, 2010.

In order to redeem your membership a consent and waiver form must be signed. If you are under the age of 18, you must have a parent come into the store and sign a consent waiver form. If you are under the age of 16, your parent must also be present in the store while you tan, on each visit. Under the age of 13, you must also have written permission from your physician.
