Each card can be used for purchases at participating locations of the business named on the face of the card. The card is not redeemable for cash, and lost or stolen cards will not be replaced except as required by law. At the merchant's option, card expires 12 months after the date purchased, except where prohibited. After 12 months of non-use, a service fee of $2.00 per month will be deducted from the card balance unless prohibited by law.

Important:  Following e-mail notification of a winning bid, this item will be available for pick up at the MDA Salt Lake City Office.  If shipment of your item is required, shipping charges will apply and will be billed to your account.  MDA will make every attempt to combine shipment of items; however, if items you purchased during this auction are located in multiple MDA offices, you may incur additional shipping charges.
