Card is not redeemable for cash, and lost or stolen cards will not be replaced except as required by law. At the merchant's option, card expires 12 monthes after the date purchased, except where prohbited. After 12 monthes of non use, a service fee of $2.00 per month will be deducted from the card balance.

Important:  Following e-mail notification of a winning bid, this item will be available for pick up at the MDA Salt Lake City Office.  If shipment of your item is required, shipping charges will apply and will be billed to your account.  MDA will make every attempt to combine shipment of items; however, if items you purchased during this auction are located in multiple MDA offices, you may incur additional shipping charges.
