The recipient by redeeming this certificate accepts and agrees to comply with all of the terms and conditions set forth on the certificate. The recipient must be a current D'Adamo Institute patient seen by a D'Adamo Institute doctor within the past 2 years. Reservations for treatments must be made in advance and are subject to availability. Supplements may be purchased only if prescribed by a D'Adamo Institute doctor within the past 2 years.
When scheduling, please state that appointment will be paid by certificate and it must be presented to front desk at time of purchase. The certificate may be supplemented by payment of cash, acceptable check or valid credit card. no change will be given on unused portion of certificate buy may be credited towards their account. Not redeemable for cash or other substitutions, not replaced if lost, destroyed, stolen or expired, and void if altered, photocopies or reproduced. Certificate will not be extended beyond the expiration date.
Cancellation policy: Cost of therapy will be deducted from face value of certificate if the scheduled therapy is missed or cancelled in less than 48 hours prior to the treatment.
