Not included as part of the prize is prescreening testing, stimulation medications, any testing performed outside Shady Grove Fertility RSC, treatment/surgery costs of side effects or complications, hospital charges, pesa/teas/tese/testicular mapping services and associated facility fees, urologist charges, social worker charges, assisted hatching, microdissection, PGD biopsy, cryopreservation and storage fees, gestational carrier fees.

 

This is a comprehensive list of exclusions at this time, there may be additional tests/programs that may be considered exclusions, as well as any other test, products or services not specifically listed herein as part of the prize. Services not listed above are not included.

 

Prize cannot be applied to the Shared Risk program.

 

Prize is transferable, but not refundable, or redeemable for cash, and no substitutions are permitted. Prize cannot be applied to any of Shady Grove Fertility’s Shared Risk refund programs. Prize may not be sold or exchanged. Federal, state and local taxes are the responsibility of the winner.

 

In the event the winner cannot use the prize prior to the expiration date, the prize will be forfeited. Upon forfeiture, no compensation will be given and no further prizes will be awarded.

 

Current patients of Shady Grove Fertility RSC must have all outstanding accounts paid in full prior to the start of the treatment cycle. Prize may not be applied to treatment in progress or past treatment.

 

Prize is applicable for one treatment cycle only, if treatment cycle is less than $9,500 there will be no refund or cash value for the unused portion of the prize.
