Mission
The mission of Connecticut Community Care, Inc. is to provide care management services to older individuals, and other persons with similar chronic care needs and their caregivers, to assist them in remaining as independent as possible in the most cost-effective manner. CCCI will continue to be the premiere provider of long-term care care management to low-income elderly in Connecticut.

CCCI has been assisting elders and individuals with disabilities and their families for more than 25 years providing care management as an access agency to the public Connecticut Home Care Program for Elders and through our private division, Care Management Associates.
